Solterra Rehabilitation/ Transitional Care
Transitional Care refers to our rehabilitative services that follow a hospital stay for an illness or injury. It is designed to expedite the healing process in a relaxed and comfortable environment. Teams–including concierge, physician, nurse, physical, occupational, and speech therapists–care for guests, enabling them to return home as quickly as possible. In addition to the rehabilitation services, teams of registered nurses, licensed practical nurses, and a physician address acute illnesses such as recovery from stroke, heart attack, injury and the exacerbation of chronic illnesses that must be stabilized before returning home. Services might include:
Physical Therapy
- Minimizing risk of falls & increasing safety and endurance
- Training in mobility, posture & gait
- Mobilizing joints and soft tissue to increase range of motion
- Using prosthesis when applicable
Occupational Therapy
- Bathing, dressing, feeding skills
- Facilitating memory, orientation and cognition
- Adapting techniques to overcome physical challenges
Speech-Language Therapy
- Developing oral muscle control & strength necessary for speaking and swallowing
- Recovering speech, language and memory skills
- Increasing nutrition and hydration
Restorative Therapy
- Increasing strength, mobility & endurance
- Promoting continence
- Preventing pressure ulcers
- Building confidence for walking and transferring
